Vaccine Records

• All pets must be current on their vaccinations.
• Dog owners must submit written proof of Rabies, DA2PPV and Bordetella vaccinations. Rabies vaccination is required for puppies over 4 months of age. We strongly encourage monthly heartworm and flea+/- tick preventative. If your pet is on a topical flea treatment, Capstar will need to be purchased to board.
• Cat owners must submit written proof of Rabies and FVRCP. We strongly encourage monthly heartworm and flea preventative.
• Vaccine records can be faxed to Tender Care Animal Hospital of Peoria at (309) 243-1758 or Tender Care Animal Hospital of Morton at (309) 266-1432.
• Due to loss and damage, we will no longer accept pet food bowls, bedding, and/or crates. Beds may be rented from Tender Care.
