Please fill out this form as completely and accurately as possible so we can get to know you and your pet(s) before you arrive.

Thank you for your interest in Tender Care Animal Hospital Doggie Daycare. Please submit one application for each dog that you would like to have in off-leash play. This application will take 10 to 15 minutes to complete.

Please note that filling out this application does not automatically enroll your dog into our daycare. After we receive your application, one of our representatives will contact you to schedule a free daycare trial.

All Dogs must be at least 16 weeks and have completed all required vaccines before attending daycare.
Puppies 4 to 6 months old NOT spayed/neutered can go into daycare as long as they are up-to-date on their vaccines. After 6 months of age it is REQUIRED of all dogs to be spayed/neutered.
Please provide a copy of all current vaccinations or allow us to contact your current veterinarian for that information. All daycare dogs are required to be up to date on rabies, distemper, and bordetella vaccinations
